Fellowship Application
We accept applications from all primary care specialty trained physicians including family medicine, pediatric, med-peds, physical medicine and rehabilitation, and emergency medicine physicians.

The application should be completed and mailed to the following address.

The AMSSM application, found here, for the Sports Medicine Fellowship should be filled out completely and returned sometime between July 1st and October 1st. Deadline for receipt of application is October 1st. The application will be reviewed by the Fellowship Selection Committee and you will be contacted to schedule an interview if you qualify.

In support of this application, we must have all of the following:

  • Letter of recommendation from your residency director (must confirm eligibility for board certification by the beginning of the fellowship year).
  • Two letters of recommendation from physicians who have supervised you in a clinical setting (should be sent directly to the address above).
  • Copy of your medical school diploma.
  • Copy of your medical school transcripts.
  • Certificate (or other validation) of all previous training.
  • Present state medical licenses.
  • Copy of all Board score reports.
  • Curriculum vitae.
  • Personal statement.
  • Florida Medical license by July 1st of Fellowship year.
  • Recent photograph.
  • Copy of ECFMG certificate.

Because we participate in the Specialty Match you must register with the National Residency Match Program (NRMP).
